<p>Ovarian Cysts After The Menopause: Reasons, Threats And Solutions</p>
<p>Even though ovarian cysts after the menopause are less common, instances do crop up and may cause difficulties. Post-menopausal women with an ovarian cyst that is not suitable for conservative management may have to have an oophorectomy. This operation is done to take out the ovary within a bag so as not to have the cyst break<span id="more-231"></span> open in the peritoneal cavity. Post-menopausal women are recommended to take a sonographical CA125 test using transvaginal grayscale. Magnetic resonance imaging (MRI), computed tomography (CT), and Doppler scans are not as good for the detection of post-menopausal cysts. Transvaginal ultrasound is the best way to understand the situation of ovarian cysts because it gives enhanced detail and more sensitivity. Larger cysts nevertheless should be examined transabdominally.</p>
<p>After menopause, ovarian cysts are contracted by about 17 percent of women. No optimal management solution for cysts exists. Many cysts will be reabsorbed by themselves without major difficulty. Malignancy and ovarian cysts do not appear to have much correlation, but ovarian cancer is showing a disturbing rise in older women. Survival is statistically unlikely, if the cancer spreads beyond the ovary. To be completely sure it is necessary to do a full laparotomy and staging procedure, even though it is well to be suspicious of the possible malignancy of all ovarian cysts in women after the menopause. From a sample of 226 women recent research on post-menopausal ovarian cysts suggests that ovarian cysts that are smaller than 50mm in diameter are benign and can be treated with safe management involving regular monitoring of the cyst size and the CA 125 levels.</p>
<p>For a woman after menopause, ovarian cysts generate two questions, the first concerning the best management and the second concerning where the treatment should take place. A typical test is the measurement of CA125 that is used in more than four out of five cases. A cutoff of 30 u/ml is used typically and the test sensitivity is 81 percent with specificity of 75 percent. The use of ultrasound has been shown to have 89 percent sensitivit<br />
1000<br />
y and 73 percent specificity. Doppler sonography with color flow has also been demonstrated to usefully assess ovarian cysts. Examining the fluid cytologically from an ovarian cyst is less effective in deciding if a tumor is benign or malignant. The sensitivity is only about 25 percent with a greater danger of breaking open a cyst. When used with an index to measure the risk of malignancy, management changes should be revised accordingly. A general gynecologist will be able to manage women with low risk, but women at an intermediate risk level should be referred to a cancer unit and those with a high risk level should be accompanied to a cancer center.</p>
<p>In the laparoscopic management of ovarian cysts in post-menopausal women, the recommendation is often for oophorectomy instead of cystectomy. Frequently the error is made in choosing ovarian cyst fluid for a cytological assessment in an effort to identify cyst malignancy. The precision factor is only 25 percent in this case and there is also the risk of the cyst disintegrating. It is the high threat malignancy index that shows all ovarian cysts in post-menopausal women, which are suspected of being malignant. If a laparoscopy indicates suspicious clinical findings, then a full laparotomy and other staging procedures are to be employed. These must be done by a surgeon qualified for this as part of a multidisciplinary team working at a certified cancer center. Therefore one may deduce that aspiration has no real role to play in the post-menopausal management of asymptomatic ovarian cysts. Nevertheless, in conjunction with laparotomy and laparoscopy it might be a step in the preliminary surgical management. The extended midline incision should comprise biopsies from areas and adhesions under suspicion, the cytology in the form of ascites or washings, BSO, TAH and infra-colic omentectomy and laparotomy that is well documented. If the cyst is malignant this may have grave further effects on the probability of the patient surviving.</p>
<p>Similar to a number of other chronic complaints, ovarian cysts after menopause are not caused by one factor only. Conventional medicine that only acts on a particular symptom will therefore not be successful in curing ovarian cysts. Several factors need to be treated in the formation of an ovarian cyst. Some of these are directly responsible for the generation of such cysts, whereas others will act to worsen cysts that already exist. A primary cause might perhaps be dealt with by conventional medicine, but the indirect factors will remain and cause complications. A holistic program is the only possibility to fully relieve yourself from ovarian cysts after menopause. The treatment needs to be multi-dimensional because of the multiple factors involved in ovarian cysts. This is the only way of getting to the underlying problems and eliminating cysts forever.</p>

Polycystic Ovarian Syndrome is one of the causes of infertility in women due to the fact that women do not ovulate. PCOS is a very complex metabolic syndrome that has multiple factors that can disrupt fertility.</p>
<p>Polycystic Ovarian Syndrome, which does not allow a women to release an egg or ovulate regularly, affect about 8-10% of women who are of reproductive age causing infertility. Ovaries may have multiple small cystic structures that are about 9mm in diameter and as small as 2mm. A mature follicle in a normal menstrual cycle with ovulation is usually 18-28mm in diameter.</p>
<p>Conceiving when you are dealing with PCOS is difficult at best. Ovulation of a mature egg may take place occasionally for some, for others they do not ever ovulate. Women under the age of 35 only have about a 15% change of pregnancy per month. However, there are various forms of prescription treatments that have been successful in helping women achieve pregnancy. Clomid along with Glucophage has been successful as a combined treatment. Glucophage is usually used for the treatment of diabetes. Insulin resistance plays a large role when it comes to PCOS. This combination therapy has been very useful in the stimulation of ovulation in some women. If this treatment is not successful in 6-9 months then there are injectable options available to you.</p>
<p>Almost 90% of women who use injectable FSH hormone therapy have been able to successfully ovulate. This therapy does have a risk of multiple births especially the risk of triplets or higher.</p>
<p>Of course there is always the option of IVF (In Vitro Fertilization). Most women with PCOS that are unable to get pregnant with other therapies have been able to achieve pregnancy with IVF.</p>
<p>PCOS is basically the result of poor communication between your reproductive system and your insulin. Insulin is trying to tell your hormones when to switch on and off but they are not listening correctly due to the PCOS creating irregular ovulation. It is possible to return your ovulation back to a normal cycle with proper hormone levels achieved through proper medical treatment. <p>Both types of cysts i.e. simple and complex can cause pain during or after the menstrual cycle. If the cyst is functional then there will be a premenstrual pain as it is reacting to the higher denseness of blood-borne hormones taking place during the stage of the menstruation. Some other reason for the pain during the period of menstruation is because having a cyst which is called as an endometrioma.</p>
<p>Growth of an endometrioma, also called as chocolate cyst or endometriod cyst. This specific condition takes place when the tissues that usually arise in the uterus originate outside the uterus, within the peritoneal cavity. <p>Many women with PCOS (Polycystic Ovarian Syndrome) are often prescribed oral contraceptives to regulate their menstrual cycle and balance their hormones. However, this does not treat the underlying cause of PCOS and once the oral contraceptives are discontinued the symptoms of PCOS will reappear.</p>
<p>Contraceptive pills can also have a detrimental effect on the treatment of PCOS. According to a recent study published in the Journal of Endocrine Regulations, oral contraceptives may actually make you gain weight. (1)</p>
<p>The study looked at 19 non-obese women with PCOS who were evaluated before and after taking Diane 35, an oral contraceptive used in many countries. Diane 35 is a combination birth control pill containing ethinylestradiol and cyproteroneacetate.</p>
<p>After the women had used the pill for 7 months, researchers noticed a significant increase in weight and elevated cholesterol levels. The weight gain in these women negated the benefits of using the birth control pills to lower the elevated testosterone levels found in PCOS sufferers.</p>
<p>This is an interesting finding because many women with PCOS already have issues with weight gain and elevated cholesterol levels. These symptoms could be exacerbated by a course of action often prescribed to help treat PCOS. However, PCOS is not the exclusive preserve of women who are overweight or obese. Up to 50% of PCOS sufferers may be of normal weight or lean.</p>
<p>Therefore, women should be aware of the benefits and risks before deciding to go on oral contraceptives for PCOS. It is important to be pro-active about your health and learn as much as possible about the treatments that you are being prescribed.</p>

<p>Polycystic Ovary Syndrome (PCOS) is a set of disorders in which the ovaries either release more than one egg or don&#8217;t release the egg cell at all. It also includes excess androgen levels and weight gain. Hair loss and excess hair growth will also affect people who have this syndrome.</p>

<p>The reproductive age woman category, now that&#8217;s the ones -if she&#8217;s trying to get pregnant, then obviously she&#8217;s not on any kind of birth control and most likely they&#8217;re functional.  What do you do?  Well you can watch them month to month, and if they resolve, which most of them will, the problem is gone.  If such a cyst grows or causes more problems, then she should also have a laparotomy.  Now, if<span id="more-209"></span> that woman &#8211; well, the other way is of course, many of these women you&#8217;d say listen, what we need to do is put you on a short course of birth control pills to see if the cysts stop forming.  </p>
<p>Birth control pills, the way they work, is they, they work on the complex mechanism that regulates ovulation, interrupting all those pathways that I briefly described earlier and that complex pathway interrupted, no more ovulation occurs, no more functional cysts occur.  And the cyst that was there will most likely resolve because of less feedback from the other hormones so birth control pills in a way you could say could cure cysts.  But these are the cysts- </p>
<p>That may not always be an option for some women that have problems with birth control pills or other side effects.</p>
<p>For those women, those group of women who cannot take birth control pills. If for whatever reason, and there&#8217;s a litany we won&#8217;t get into here of women who cannot take the pill for one reason or the other, then it would be expected management, that&#8217;s basically just watching and seeing, cause most of these will go away within three menstrual cycles.  If they do not, then, then most likely she&#8217;s going to have a laparotomy.  Obstetrician Gynecologist, Christopher Freville, recommends you could also do a laparoscopy.  A laparoscopy again is a very common term, this is where a telescope like tube with a light on it is put in, the doctor is able to look through an incision in the naval, while of course I  might add the patient is asleep and fully anesthetized, is able to look at the pelvic organs.  But if the doctor sees a six-centimeter cyst, he is going to operate anyway, so most of the time that would be a procedure that would not have a purpose.  She needs a laparotomy to make sure that this particular ovary cyst isn&#8217;t one of the other cysts, of course the endometriomas, the sustatnomas, the dermoid cysts, the polycystic ovaries, which usually aren&#8217;t going to show up as large cysts.</p>
<p>And of course, the last category is the rarer cystic tumor that would be cancerous.  So, the management basically is one of just, in most women is, in the reproductive age group is going to be wait and see.  You know, reassurance, come back in a month.  You can take, if the cyst goes away, great, if it doesn&#8217;t or if it slowly resolves fine, if it doesn&#8217;t go away, you can offer a course of birth control pills.  If that&#8217;s not acceptable or not possible, then the last way is to of course to actually take a look through an incision, a laparotomy.  </p>
<p>And that I think that pretty much sums up the actual treatment of cysts, at least as far as a diagnostic program.  There are other treatments of cysts, and I will mention these each by the category.  So we got a functional cyst that are self-resolving or you&#8217;re going to use birth control pills.  We&#8217;ve got the endometriomas which begin the endometriosis on the ovary.  This is a case that needs to be treated with various medications.  There are medications to suppress ovulations, stop the period which burns up in the endometriosis and it&#8217;s generally about 90% effective, and also causes fertility when you get these to go away.  So the chocolate cysts once they are, they&#8217;re usually removed at laparotomy, you can also cauterize them, that&#8217;s zapping them with electricity.  Again, the patient is asleep because the endometriomas and endometriosis are usually multiple places in the pelvis, and then follow up with medications to suppress ovulation.</p>

I put on a lot of weight due to thyroid problems and also PCOS which causes rapid weight gain, so how can you be so negative about these women if you do not know their individual cir$%^&#038;stances?
